👤 📄 Required Documents for Opening a Jan Aushadhi Kendra (PMBJP)

✅ 1. Applicant Documents

  • Aadhaar Card
  • PAN Card
  • Passport size photographs
  • Mobile number & Email ID

If applying as:

  • Individual – Personal documents
  • NGO / Trust / Society – Registration certificate
  • Company / Firm – Incorporation certificate, GST (if applicable)

✅ 2. Pharmacist Documents (Mandatory)

  • Registered Pharmacist Certificate
  • D.Pharm / B.Pharm certificate
  • State Pharmacy Council Registration
  • Appointment letter (if pharmacist is hired)

⚠️ A registered pharmacist is compulsory to operate the store.

✅ 3. Shop / Premises Documents

  • Proof of ownership (Sale deed / Property tax receipt)
    OR
  • Rent agreement (if rented shop)
  • Minimum area required:
    • 120 sq. ft. (minimum)
  • Shop photos (inside & outside)

✅ 4. Drug License

  • Retail Drug License issued by State Drug Control Department
    (Forms 20 & 21)

✅ 5. Other Required Documents

  • GST registration (if applicable)
  • Bank account details & cancelled cheque
  • Affidavit as per PMBJK format
  • Undertaking form

  • SC / ST applicants – Caste certificate
  • Divyang applicants – Disability certificate
  • Women entrepreneurs – ID proof (for preference)
